Creative Mind Child Development Clinic for High Risk follow up
High-risk newborns’ are defined as those requiring resuscitation at birth, those with neonatal encephalopathy, prematurity, low birth weight, jaundice, hypoglycaemia, neonatal meningitis/sepsis, neonatal seizures and congenital anomalies
• Surveillance and Screening:
-Developmental surveillance at every well-child visit
-Developmental screening using a standardized screening tool at 9, 18, and 30* months or when concern is expressed
-If screening results are concerning, developmental and medical evaluations and early intervention services
-Regular follow up visits as advised to track child’s developmental status
• Early intervention, Diagnosis and follow up: Multidisciplinary services provided to children from birth to 5 years of age to promote child health and well-being, minimise developmental delay, remediate existing or emerging disabilities, prevent functional deterioration and promote adaptive parenting and overall family function.
